  2. Curriculum Vitae (CV)
  3. Scan of Passport (in JPEG, see the example)
  4. Scan the Insurance Policy Document
  5. Scan of completion certificate of the last educational background
  6. Scan of academic transcript for the last educational background
  7. Scan bank statement showing a minimum balance of USD 2000
  8. Soft file Photo 3 cm x 4 cm

The required documents for Visiting Professor/Speaker (less than 2 months): VISA C10

  1. Application Form (download)
  2. Curriculum Vitae (CV)
  3. Scan of Passport (in JPEG, see the example)
  4. Scan the Insurance Policy Document
  5. Scan bank statement showing a minimum balance of USD 2000
  6. Soft file Photo 3 cm x 4 cm
  7. Invitation Letter

  2. Scan KTP Pendamping
  3. Foto Pendamping 3 cm x 4 cm
  4. Kontrak/MoU
